PLEASE KEEP DETAILS BRIEF. POINT FORM AND ONLY THE FACTS DIRECTLY SUPPORTING YOUR CASE
- Include your Full Legal Name, Phone Number(s), Address, City, Postal Code, E-mail and SIN (mandatory for Ontario Works and ODSP).
- State your DESIRED OUTCOME (exactly what you would like us to do for you)
- List any Case Numbers your have been given and the Agency that assigned them
- List all the people that you have discussed your case with, their phone numbers, extensions, e-mail, Agency and what they have told you to date.
- You can expect to sign an Authorization Form allowing us to make enquiries on your behalf.
- You can be assured we will call you regarding any questions we may have and for a regular update on how we are progressing with your case.
